Making an Appointment
To make your first appointment, call (415)
750-5513 or (415) 750-5508. We will ask you a few questions
about your diabetes and make your appointment. When you call
you may reach the message center. Please leave your name and
number so that we can return your call. We appreciate your
patience.
After your first appointment please call (415)
750-5517 to make or change appointments.
First appointment: bring your blood glucose
meter if you have one, and your medications or a list of your
medications.
Number of appointments is individual, based on
your needs. Your insurance may only cover a specified number
of appointments.
Appointments are available Monday through
Friday.
Individual
Sessions
Individual sessions with the
dietitian or nurse diabetes educator are by
appointment.
Learn how and when to check your blood sugar levels, about your
medications, and about how to keep healthy with diabetes. Learn
what foods affect your blood sugar and design your personal meal
plan.
You are welcome to bring a friend or family member with you.
There is no one right way to deal with
diabetes; certified educators will work with your doctor to make
sure you have a plan that is right for
YOU.
Registration
Register on the 5th floor 10 minutes before
your appointment.
You do not have to register when you come to the group sessions on
the 4th floor.
