The Case Management Department at St. Mary's Medical Center consists of a highly skilled team of Nurse Case Managers and Social Workers.  They work closely with the clinical team to assess a patient's needs, assist with discharge planing and address any concerns the patient or family may have.  Our primary goal is to help arrange a safe and satisfactory discharge and educate patients in all of their options and available resources.

Hours:
Monday through Friday, 7:30 a.m. to 4:30 p.m.
Saturday, 8 a.m. to 4:30 p.m.
