Before you are admitted to St. Mary's Medical
Center or receive outpatient services at the hospital, you will
need to complete the registration process.
Inpatient
Admissions
If you are being admitted to the hospital, please come to the
Admitting Department on the Lobby Level. Once you've completed the
registration process, you will be escorted to your hospital
room.
What to
Bring:
- Your insurance card
- Orders or forms from your doctor
- If you have completed an advanced health
care directive form, please bring a copy with you to
Admitting.

Monday through Friday
6:30 a.m. to 5 p.m.
After business hours, and on weekends and holidays, please go to
the Emergency Room Admitting Area on Level A.
Outpatient Services Registration
Hours
Monday through Friday
7:30 a.m. to 6 p.m.
If you are receiving medical services as an
outpatient, go to the Outpatient Registration Area on Level A (one
floor below the Lobby Level). Once you register, you will be
directed to the appropriate department to receive the medical
services or testing your doctor ordered.
If you are receiving radiology services, you
can go directly to the Radiology Department to register. You do not
have to visit the Outpatient Registration Area on Level A.

Register over the
Phone
For added convenience, you can complete the registration process
over the phone for all outpatient services except laboratory
services. Call (415) 750-5705 to register. Please have your
insurance card with you when you call.
Outpatient surgery
To register for outpatient surgery, go
directly to the Surgical Unit on 3-East between 8:30 a.m. and 5:00
p.m. Monday through Friday. You can complete this process up
to two weeks before your scheduled surgery. No appointment is
necessary, but it is highly recommended. Please bring your
insurance card and any forms or orders from your doctor's
office.
During your pre-admission appointment, you
will meet with the Surgical Nurse Facilitator who will review your
health history and give you detailed instructions for the day of
your surgery and answer any questions you might have. If you need
any lab, X-ray or EKG tests, these can be done during your
visit.
If you are unable to pre-register in advance,
the Nurse Facilitator will call you one to three days before your
surgery to advise you about pre-surgery instructions. You will also
be connected to the Admitting Department to complete the
registration process.
