Insurance coverage for diabetes education varies widely. Please call your insurance company and ask about coverage for "diabetes education" or "diabetes training."
Some insurance companies require pre-authorization.
Brown and Toland Medical Group and Hill Physician Medical Group require a referral form from your physician. You may ask your physician to mail or fax the form or you may bring it to your appointment.
Medicare covers appointments.
