Referrals and Admissions
The Acute Rehabilitation unit accepts referrals from physicians, social workers, rehabilitation nurses, case managers, patients and their families. Potential patients receive a free assessment to determine their appropriateness for the program. For additional information or to make a referral, call (415) 750-5932.
