Discharge Planning
Getting patients home is our primary goal. At St. Mary's, a patient's care does not end at discharge. Our nurses, therapists, social workers and case managers work with patients and their families to make sure their return home goes smoothly. Our rehabilitation team educates family members about any continuing therapy and home support a patient will need, conducts home evaluations when indicated and arranges for adaptive equipment needs.
To continue progressing outside of the hospital, our team will coordinate appropriate ongoing rehabilitation services. Many patients in the San Francisco Bay Area continue to receive their therapy through our outpatient rehabilitation program at the Ben Berman Outpatient Rehabilitation Center.
