If you are caring for a frail elder or disabled adult at home, St. Mary's Medical Center offers a community program that provides therapeutic care on a daily basis for your loved one.
The Adult Day Health Care Program provides health maintenance and rehabilitation services for adults and elderly patients with disabling medical conditions. Our program strives:
- To restore and maintain each participant's optimal level of health and independence
- To delay or act as an alternative to long-term institutional placement
- To provide respite and additional resources for caregivers
In a comfortable, neighborhood setting, participants can improve the quality of their lives by learning to maintain their independence to continue living in the community. People who come to Adult Day Health Care tend to experience increased independence, friendships and companionship and suffer less from depression than people who are isolated at home. The atmosphere inside the walls of the Center is filled with genuine love and compassion by a team of caring professionals.

Our multidisciplinary health care team develops an individual plan of care for each participant. This plan is based on a medical treatment plan provided by the participant's primary care physician as well as the team's assessment of the participant's medical conditions and needs.
Caregiver Support
St. Mary's Adult Day Health Care Program helps relieve the emotional and physical burdens placed on families and caregivers by providing care for the adult with disabilities during the day. The program offers caregivers the respite they need to find time to enjoy life again.
We also run a monthly caregiver support group, which is open to the community at large. This is a great opportunity for caregivers to meet new friends, share stories and learn new coping strategies for caring for someone at home.
Who Comes to Adult Day Health?
Our variety of services benefit individuals who are in need of continuing medical or rehabilitative services related to conditions such as: stroke, diabetes, Parkinson's disease, respiratory or cardiac disease, multiple chronic illnesses and Alzheimer's disease.
Hours
Monday through Friday 8:30 a.m. to 5:00 p.m. Participant hours are 9:30 a.m. to 2:30 p.m.
Cost
Adult Day Health Care services are covered by Medi-Cal, VA and private payments. There is a sliding fee scale for private payers.
Location
Located in the Outer Mission, St. Mary's Adult Day Health Care serves residents of the Ingleside, Ocean View, Merced Heights, Alemany, Cayuga, Excelsior, Mission, Outer Bernal Heights, Crocker-Amazon and Visitacion Valley neighborhoods. Our multicultural staff speaks Mandarin, Cantonese, Spanish, Tagalog and Russian.
