Once your physician has informed you that you
can be discharged, please plan that the processing of your
discharge orders will take 2 to 3 hours. This time is needed to
assure that you receive all the necessary information and education
to ensure your safe discharge and that all consulting physicians
who have been involved in your care are notified. Discharge time is
11 a.m. Please arrange for someone to be available to take you
home by 11 a.m. If you cannot leave by this time, please notify
your nurse so alternate arrangements can be made.
While you are in the hospital there will be a
case manager available to assist you with your discharge planning.
Your physician, nurse, and case manager will give you information
needed to continue your recovery at home. If you have questions
about diet, medications, the time of your next physician's
appointment or what continuing care is needed at home, ask your
physician or nurse.
Discharge
Instructions and Medications
Your physician and your nurse will give you instructions about
post-hospital care. If you have questions about your diet,
activities or other matters, speak with your nurse. If your
physician has prescribed medications, you will be given a
prescription, which you may have filled at your local pharmacy.
Skilled Nursing
Facility
There are occasions when patients need additional treatment or care
after they are discharged from the hospital. St. Mary's Skilled
Nursing Facility is designed for patients who no longer require
acute care in a hospital but who may have additional skilled care
needs such as physical therapy or specialized nursing care.

Home Health
Care
Your physician may request
continued in-home services after you leave the hospital. The
case manager will help make the appropriate arrangements for those
services you require in the home. This may include skilled nursing
care, physical therapy, or private attendant care.
Senior
Services
The Senior Services Department provides information and community
referral resources to help support safety, independence and the
health of seniors. These services are non-medical and can include
transportation, meal programs, and assistance with Medicare and
insurance issues as well as Lifeline services. For more

Financial
Arrangements
The hospital's financial advisors confirm insurance benefits prior
to or immediately after admission. Payment for estimated charges
not covered by insurance are requested at the time of discharge.
This includes charges such as insurance deductibles, co-payments
and guest meal trays.
